PP-251: SURGICAL TREATMENT OF POSTINFARCTION VENTRICULAR FREE WALL RUPTURE

Objective: A cardiac rupture in patients with AMI is the second most common cause of hospital death, after pump failure. It complicates approximately 4% of myocardial infarctions and accounts for 12% to 21% of deaths after infarction. Surgical repair is mandatory even with high operative mortality. The optimal surgical technique is controversial since the results depend on type of rupture. We present our mid-term surgical experience according to the status of the ventricular tear and type of surgical repair. Methods: This retrospective study included 7 patients (2 females (28.5%), 5 males; mean age 63 years; range 40 to 80 years) who underwent Surgical treatment of postinfarction ventricular free wall rupture at Kartal Kosuyolu Research and Education Hospital between 1997 and 2009. Post infarction ventricular free wall rupture was seen at left ventricle in 5 (71.4%) patients, at right ventricle in 2 (28.5%) patients. Results: 5 patients (71.4%) were repaired with cardiopulmonary bypass. Pericardial patch repair was performed in 2 (28.5%) patients and primer repair was performed in 5 (71.4%) patients as surgical procedure. CABG was performed in 2 (28.5%) patients. In hospital mortality was 2 (28.5%) patients. Conclusions: In patients with ventriclerupture, early diagnosis and surgical treatment are crucial for successful outcome.
