Fertility care for human immunodeficiency virus–positive couples: supply and demand

In the developed world, fertility care for the HIV-infected couple needs to be optimized by making a formal assessment of demand and rationing provision to a small number of dedicated but geographically accessible centers able to offer cost-effective, risk-reduction treatments that minimize viral infection risk to the uninfected partner and unborn child. Multicenter pooling of data should be encouraged as a means of establishing evidence-based guidelines.
