Abstract Percutaneous coronary intervention (PCI) remains necessary in patients with spontaneous coronary dissection (SCAD) with ongoing ischemia. However, PCI in SCAD is associated with poor results. Fenestration with a cutting balloon has been described to release the extraluminal compression. The authors describe 2 cases managed successfully with another fenestration technique—the subintimal tracking and re-entry technique. (Level of Difficulty: Advanced.)
