Chronic ankle instability is a frequent condition which might require surgical treatment. It affects people of all ages and levels of activity but it has been more frequently linked to younger, more active, sports-active population. There is no consensus concerning the decision for conservative versus surgical treatment, and definitely no consensus on the choice for the type of surgical repair when indicated. Herein, it is described as the surgical option of using a periosteal fibular flap technique which has been used in skeletally immature patients and high-level athletes with favorable results and has proven to be a valid option as augmentation or even compared with the possibility of reconstruction by means of an allograft.
