EVALUACIÓN DE LA ESTRATEGIA DOTS EN SANTIAGO DE CALI

Objetivo: Evaluar la situacion actual de la estrategia DOTS frente a, compromiso politico, acceso a la microscopia de esputo, acceso ininterrumpido de farmacos, registro y notificacion de casos y quimioterapia acortada supervisada, en Cali 2007. Materiales y Metodos: Estudio descriptivo de corte transversal. Seleccion: 30 IPS de manera aleatoria y diferente nivel de atencion. Muestra por conglomerados monoetapica (IPS). Resultados: 60% de las IPS no conocen la estrategia DOTS, 73% no tienen consultorio para los pacientes con tuberculosis (TB), 53% no realizaron busqueda activa de casos en el ultimo ano y tan solo 20% ordenaron realizar baciloscopia (BK) a los sintomaticos respiratorios, 47% muestran deficiencias en implementos de bioseguridad y 100% usaron el tratamiento acortado supervisado (TAS), pero con dificultades en la entrega de medicamentos. Conclusiones: Se identifico que existen dificultades para el cumplimiento de los componentes de la estrategia DOTS en Cali. Abstract Objective: To assess the current situation of the DOTS strategy in terms of political commitment, access to sputum smear microscopy, uninterrupted access to drug, recording and reporting of cases and supervised short-course chemotherapy in Cali, 2007. Materials and Methods: Cross-sectional descriptive study. Selection: 30 IPS1, with different levels of care, at random. Single-stage cluster sampling (IPS). Results: 60% of the IPS are not aware of the DOTS strategy, 73% have no clinic for patients with tuberculosis (TB), 53% did not conduct an active case search in the last year and only 20% ordered a bacilloscopy (BK) for respiratory symptoms, 47% showed deficiencies in biosafety equipment and 100% used the directly observed treatment short course (TAS, for its acronym in Spanish), however, with difficulties in medication delivery. Conclusions: There are difficulties in the implementation of the DOTS strategy components in Cali.
