Objective: Verify the association between the systemic arterial hypertension, cognition and mental capacity in the elderly. Methods: Systematic review made between February of 2019 and April of 2020 of quantitative studies searched on PubMed, BVS health and Scopus, that evaluate the relationship between high blood pressure and cognition in seniors (> 60 years old), based on the PICO question. Inclusion and exclusion criteria were predefined and the articles were selected by the authors in an independent manner in two stages. The results were analyzed and described from the data extraction table. Results: Among the selected studies, there were no homogeneity when it comes to the description of the pressoric values of the patients. Furthermore, there is no details about the anti-hypertensive treatment. So, the evidences about the relationship between the cognitive impairment and hypertension would need more studies for your comprovation, since there is variation between the variables. However, studies in this review make points that shows the hypothesis of this relationship. Final considerations: This review brings arguments that demonstrate the relationship between hypertension and cognitive impairment and mental capacity in elderly, especially related to commitment of memory and cognition. That association may appear due to the increased of pulse pressure and the variation of the blood pressure that attend with hypoperfusion of the white matter.
