Background: Elderly is someone who is aged ≥ 56 years old and experiencing physical or psychological deterioration, elderly in urban areas are less than in the countryside. The elderly need family support so they don't experience anxiety. Anxiety risk of 1.2 times for not engaging in sexual activity. The purpose of this study was to determine differences in anxiety, family support, history of illness and sexual activity in the elderly (a comparative study between rural and urban areas in West Kalimantan). Method: The population was 8,714 people with a sample of 61 people using the comparative or comparison method. The sampling technique uses purposive sampling using unpaired t-test, with 95% confidence. Results: The study showed that there were differences in anxiety, history of illness and sexual activity of the elderly living in cities and those living in villages and there were no differences in the support of elderly families living in cities and those living in villages. Suggestion: Elderly is a family responsibility, so maintaining the health of the elderly is very important. One search for families to support the elderly is to motivate and facilitate the elderly so they can carry out their daily activities properly and diligently.
