Mechanostat theory postulates a feedback system to explain the adaptation of bone to the largest strains induced by muscle contraction and gravitational forces exerted by body weight. Exercise becomes less effective to increase bone strength after puberty, reflecting a loss of bone sensitivity to strains. Weight-bearing activities and resistance training appear effective to reduce hip and spine postmenopausal bone loss. Whole body vibration is a new promising technique to reduce the risk of osteoporotic fractures since it may result in a gain of bone mineral density and balance in frail older subjects.
