The article provides information about the new technology of assessment of the whole blood hemostatic potential in the process of coagulation – low-frequency thromboelastography (LFTE) – which allows integral assessment of the hemostatic potential. The LFTE possibilities are illustrated by evaluation of the effects of antiplatelet drugs (COX-1 inhibitor – acetylsalicylic acid, platelet ADF receptor blocker – lopidogrel) and anticoagulants (unfractionated heparinsodium heparinate, low-molecular-weight heparin – enoxaparin) and the prompt and personalized evaluation of drug pharmacodynamics. New data about specific and pleiotropic effects of antithrombotic drugs were gained. A specific response of the hemostatic potential to antiplatelet therapy is reduction of the aggregation activity of blood cells, which is manifested by enhanced suspension blood stability and decreased contact-coagulation intensity. The pleiotropic effects of antiplatelet drugs are manifested by the tendency of hemostatic potential to change toward structural and chronometric anticoagulation. Results of evaluation of the pharmacodynamics of anticoagulants allowed determining the time of their maximum impact on the hemostatic potential and the duration of anticoagulant effect. A specific response of the hemostatic potential to the infusion of anticoagulants is the development of structural and chronometric hypercoagulation that is realized by reducing the proteolytic phase of fibrinogenesis and activity of lateral fibrin assembly processes.
