Les Effets Néfastes des Mutilations Génitales Féminines / Excision sur les Fonctions Sexuelles

RESUME But. Decrire les effets nefastes des mutilations genitales feminines / excision (MGF/E) sur les fonctions sexuelles. Patientes et methodes. Notre etude s'est deroulee dans le service d’obstetrique CHU Point G. Il s’agit d’une etude cas / temoins. Le cas etait la femme excisee et le temoin, la femme non excisee. Notre etude a dure seize mois, du 01/01/2019 au 30/04/2020. L’etude a concerne les primipares de 14 a 49 ans excisees ou non. Aucune femme non consentante n’at ete retenue. Le recueil des donnees a ete fait sur questionnaires individuels. Resultats. Parmi les 1518 accouchees de la periode d’etude, il y avait 266 femmes porteuses de MGF/E ; elles ont fait l’objet de notre travail. La tranche d'âge la plus representee etait  celle de 14 a 19 ans, soit 51,1% chez les cas et temoins. Les bambaras (34,6 %) et sonrhais (31,1%) etaient les ethnies  majoritaires dans les deux groupes.. Les meres analphabetes (57,8%) et celles du niveau secondaire (59,3%) etaient majoritaires chez les cas et les temoins. L’excision de type 3 a represente 12 % des cas. Il y a eu 78,9% de complications psycho-sexuelles chez les femmes excisees contre 7,5% chez les non excisees et cette difference etait statistiquement significative (P<0,05). Conclusion. Les MGF sont une pratique frequente au Mali et sont une source notable de complications psycho-sexuelles. Des mesures doivent etre prises pour la prevention et l’eradication de ce fleau. ABSTRACT Aim. To describe the harmful effects of female genital mutilation / excision (FGM / E) on sexual functions. Patients and methods. Our study was carried out in the obstetrics department CHU Point G. It was a case / control study. The case was the excised woman and the control, was the non-excised woman. Our study lasted sixteen months, from 01/01/2019 to 04/30/2020. We included, primiparas from 14 to 49 years old excised or not. All  women participating to the study gave their consent. The sample size was calculated based on the case / control sample. Data collection was done on individual questionnaires. Results. Among 1,518 deliveries during our study period,  there were 266 women with FGM E. The age group most represented was 14-19, or 51.1% in the cases and controls. The Bambara (34.6%) and Songhai’s (31.1%) were the main ethnic groups in the cases and the controls. Illiterate mothers (57.8%) and those limited to secondary level education (59.3%) were mostly represented.  Type 3 circumcision accounted for 12% of cases. There were 78.9% of psycho-sexual complications in the excised women against 7.5% in the non-excised and that difference was statistically significant (P <0.05). Conclusion. FGM are still frequent practice in Mali.  They are cause significant psychosexual complications. Measures must be taken to control this situation.
