Die ambulante Video-Urethrozystoskopie

In 2œ Jahren wurden 785 Video-Urethroskopien vor allem wegen Tumornachsorge, Mikrohamaturie, subvesikaler Obstruktion, rezidivierendem Harnwegsinfekt und tumorsuspekter Blase durchgefuhrt. Dabei gelangte die Kamera OTV-S2 in Verbindung mit dem Adapter AR-TF 2 und einer Xenon-Lampe (300 W) der Firma Olympus zur Anwendung*. Die ersten 560 konsekutiven Endoskopien mit starrem Instrument innerhalb von 2 Jahren wurden vor allem im Hinblick auf Indikationen, Morbiditat und Akzeptanz ausgewertet. Die Gesamtmobiditat lag bei geringer Symptomatik mit 28 % niedrig und war vergleichbar mit derjenigen, die von anderen Autoren fur die flexible Zystoskopie ermittelt wurde. Bei 87,3 % der Patienten war die Akzeptanz hoch und bei nur 2,9 % nicht vorhanden. Neben historischem Ruckblick werden vor allem die Vorteile der Video-Steuerung fur Untersucher und Patienten sowie die Moglichkeiten im Rahmen der Ausbildung, des Video-Konsils und des kunftigen Einsatzes in der Tele-Medizin diskutiert. Insgesamt kann die Video-Steuerung als erhebliche Verbesserung des Zystoskopiestandards angesehen und ihre routinemasige Anwendung in der Klinik oder Praxis unbedingt empfohlen werden! Within 2œ years, 785 urethrocystoscopies were performed using video-monitoring for bladder tumor follow-up, microhematuria, subvesical obstruction, chronic urinary tract infection and suspicion of bladder tumor. The microchip camera OTV-S2 in combination with the beamsplitting adapter AR-TF 2 and a high-powered (300 W.) xenon lamp were applied*. The first 560 consecutive patientes treated with rigid cystoscopy within two years were evaluated regarding indication for endoscopy, morbidity and acceptance. General morbidity was low with symptoms in only 28 %, and were comparable to those reported using flexible cystoscopy by other authors. Acceptance was excellent in 87.3 % of the patients. In addition to a short historical review, the advantage of video-monitoring for urologists and their patients, teaching purposes, video-conderences as well as future perspectives of this technique are discussed. Generally, video-monitoring can be regarded as an important improvement in outpatient cystourethroscopy and routine application is recommended.
