La gran paradoja de la sanidad pública española: Una consideración obligada

espanolLa sanidad publica constituye uno de los pilares del bienestar socio-economico de toda sociedad moderna. El proposito de nuestro trabajo consiste principalmente en poner de manifiesto el hecho de que existe una gran paradoja en el sistema sanitario publico espanol, y ello es debido a que por su reconocida calidad se encuentra en el puesto no 8 en el ranking mundial en el que figuran 195 paises, por una parte, al mismo tiempo que posee un mercado laboral sanitario que podriamos calificar sin ambages de desastroso, injusto, e ineficiente, por otra. Sabemos que detras de toda paradoja hay siempre un enigma, y tambien que en ocasiones “la verdadera solucion del enigma es que no hay enigma”. Partiendo de esta base, y tras una breve introduccion, abordamos los epigrafes o apartados siguientes: una vision global del mercado de trabajo en Espana, una sintetica referencia al peso de la sanidad publica en la economia espanola, una informacion basica sobre el numero de profesionales de la sanidad, considerando tanto la totalidad de colegiados, como los pertenecientes a la sanidad publica, un analisis del Sistema Nacional de Salud, la contratacion laboral en la sanidad publica, tema clave en el desarrollo de nuestro trabajo, la evaluacion de la calidad y eficiencia de nuestra sanidad y su control, volviendo, finalmente, a la paradoja y al enigma subyacente en este tema transcendental con el animo de ofrecer una respuesta y explicacion convincentes. EnglishThe public health constitutes one of the pillars of the social welfare of any modern society. The aim of this work consists of showing the existence of a great paradox in the Spanish public system of health, given the fact that, from this point of view, is a country situated in the 8o place among 195 countries in the world, and at the same time has a disastrous, unjust, and inefficient health labour market. We know that behind a paradox there is always an enigma, and sometimes is also true that “the verity solution of the enigma is that there is not enigma at all”. On this basis, and after the introduction, we address the following sections: the labour market in Spain and its problems, a brief reference to the weight of public health in the Spanish economy, an overview of the number of registered health professionals, an analysis of the National Health System as a fundamental axis of public health in our country, the many and serious problems existing in the peculiar “chosen” contract model for public health professionals,-the central part of our work-, the evaluation of the quality and efficiency of Spanish public health and its control, and finally a return to the paradox and the underlying enigma in this so transcendental problem in order to give a convincing answer or explanation.
