Adequate surgical treatment of rectal carcinomas is the basis for every adjuvant therapy. Even under the most optimal conditions operations for rectal carcinomas in the stage II or III local recurrence rates of 20 to 30% and a 5-year survival rate of approximately 40 to 60% are to be expected. A combined adjuvant therapy can enhance this prognosis. Therefore the St. Martinus Hospital in Olpe, FRG, treats patients with advanced rectal carcinomas according to the FOGT-2 study. The given equipment of a rural general hospital renders performance of adjuvant therapy difficult, however does not make it impossible by means of an engaged interdisciplinary cooperation with a specialised tumour center. Patients acceptance is given and is not substantially detracted by toxicity or side effects during the 12 months duration of therapy. Affirmative data, however, can only be expected upon termination of the FOGT-2 study.
