Clinical study of severe hepatitis and liver cirrhosis combined with nosocomial infections after hepatitis

OBJECTIVE To discuss the clinical risk factors of severe hepatitis and liver cirrhosis combined with nosocomial infections after hepatitis so as to provide basis for the clinical prevention of infections.METHODS The clinical data of 312 cases with severe hepatitis or liver cirrhosis were analyzed retrospectively,and the relationship between the incidence of the nosocomial infections and the disease type,gender,age,hospitalization duration,invasive procedure,albumin level,white blood cell count,or other complication was observed.RESULTS Totally 80 cases were combined with nosocomial infections in 312 cases with severe hepatitis or liver cirrhosis,the infection rate was 25.6%.The infection rate of the patients with more than 40 years of age was 32.1%,significantly higher than 19.0% of the patients with less than 40 years age(P0.05).The infection rate of the patients with the hospitalization duration more than 20 days was 37.6%,significantly higher than 17.6% of the patients with the hospitalization duration less than 20 days(P0.05);the infection rate of the invasive procedure group was 28.9%,significantly higher than 18.2% of the non-invasive procedure group(P0.05);the infection rate of the patients with albumin level higher than 28 g/L was 31.8%,significantly higher than 21.1% of the patients with the albumin level lower than 28 g/L(P0.05);the infection rate of the patients with other complication was 30.5%,significantly higher than 20.0% of the patients without other complications(P0.05);however,the difference between the infection rate and the disease type,gender or white blood cell counts was not statistically significant.CONCLUSION The incidence of infections in the patients with severe hepatitis and liver cirrhosis is closely correlated with the age,hospitalization duration,invasive procedure,albumin level or other complications,which provides the basis for the clinical prevention of infections.
