A case of toxic epidermal necrolysis initially affecting the skin site of radiation therapy for an intra-cranial post-transplantation lymphoma

52歳の男性のpost-transplantation lymphoma患者に生じたTEN(toxic epidermal necrolysis)を報告した。患者は慢性糸球体腎炎に対する腎移植施行後，頭蓋内EBV-related malignant lymphomaを発症した。腫瘍摘出前からのフェノバルビタール(フェノバール®)内服と放射線併用療法中に，放射線照射部位を初発として多型滲出性紅斑が出現し全身性に拡大，体表面積の約35％に及んだ。フェノバルビタールを含む全使用薬剤および放射線照射の中止とステロイドセミパルス療法(ソルメドロール®の500mg/日連続3日間点滴静注)を含む治療が奏効し色素沈着を残して治癒した。
