Lymphoepithelial carcinomaa (LEC) are responsible for approximately 0.4% of malignant tumors of the salivary gland and are a variant of undifferentiated carcinoma with characteristic dense lymphoid stroma. A strong association has been demonstrated between the lesion and Epstein Barr Virus (EBV) infection. We present a case of a 42 years-old man with bulging in the left parotid region of about 6 cm with slow and progressive growing. Total parotidectomy was performed. The pathological diagnosis was multinodular parotid lymphoepithelial neoplasm. The surgical treatment associated with postoperative radiotherapy is the main treatment for LEC of the salivary glands. Postoperative radiation therapy to the tumor bed and ipsilateral neck because is recommended, since LEC are highly radiosensitive, and radiation therapy can completely eradicate microscopic residual disease. Neck dissection is reserved for patients who have clinically evident cervical lymph nodes.
