Acute kidney injury (AKI) is a common intensive care problem with an increasing incidence and a high morbidity and mortality. Due to the lack of specific therapeutic options for AKI it is prudent for the intensivist to prevent the occurrence of severe renal impairment. Early recognition of patients at risk for renal impairment and rapid consultation with a nephrology consultant are the cornerstones for the treatment of these patients. Certain uncommon but easily reversible causes should always be ruled out. Maintenance of mean arterial pressure and renal perfusion by adequate volume replacement and catecholamine therapy as well as minimizing nephrotoxin exposure are the core components for preventing kidney injury.
