Back pain is determined by many factors and may have a multitude of causes. Quite often it is not possible to establish a definite causal link between the pattern of complaints on the one hand and the results of physical examination on the other. This applies likewise to psychosocial. Assessment and/or imaging procedures. Hence it is necessary, first of all, to find out whether the acute back pain is due to a harmful and specific disease or whether it is of unspecific origin. Essential clues are supplied by history anamnesis and physical examination . Imaging methods should only be applied if they appear to promise therapeutical consequences. Therapy aims not only at pain relief so that the patient can resume his daily chores. Over and above this it is absolutely essential to avoid chronicity of the pain. This can be best achieved with a guideline-based treatment strategy. Treatment of chronic back pain should be primarily based on a multimodal interdisciplinary approach considering the biopsychosocial model of chronicity.
