Septorhinoplasty

A majority of the patients with external nasal deformities seek an aesthetic improvement of their noses. The external nasal deformities are mostly associated with a deflected nasal septum causing nasal obstruction requiring a one stage corrective septorhinoplastic operation. In this paper the techniques of doing rhinoplastic and septoplastic surgery in 406 cases are discussed. In septoplasty, incising the deflected septal cartilage in the shape of squares or excising a vertical oval strip to break the spring action of the deviated septal cartilage is found quite effectuve. In the tip plasty, the removal of a triangular shaped piece from the lateral crus of the lower lateral cartilage is suggested to achieve an esthetically pleasing shape of the nasal tip. The use of autologous and preserved homologous septal cartilage has been found quite satisfactory for the correction of slight to moderate saddle shaped nasal deformities. For reconstructive rhinoplasty the midline and off midline flaps were found useful.
