E.E.G. studies in 22 children with sulphatide lipidosis (metachromatic leucodystrophy).

SUMMARY Clinical and EEG studies are reported on 22 children with histologically proven sulphatide lipidosis. The onset of symptoms, the timing of various investigations and the course of the individual illness are considered in relation to the evolution of the EEG features. In two children (siblings of an already affected patient) in whom the EEG studies began before the onset of clinical symptoms, the EEG changes were already obvious (one and five years respectively) before the onset of clinical symptoms. In sulphatide lipidosis, the EEG features show peculiarities which are substantially different from those seen in other known inborn errors of metabolism affecting the CNS before its full maturation. It is suggested that repeated EEG investigations during the course of an obscure neuro-logical illness in young children may offer an early sign in the differential diagnosis. RESUME Etudes EEG chez 22 enfants avec sulfatide-lipidose (Leucodystrophie-metachromatique) Les auteurs rapportent les etudes cliniques et EEG de 22 enfants atteints de sulfatide-lipidose histologiquement demontree. Le debut des manifestations, la chronologie d'examens varies et le developpement individuel de la maladie sont etudies en relation avec l'evolution des caracteristiques EEG. Chez deux enfants (dans la fratrie d'un malade deja atteint) chez qui l'etude EEG debute avant le commencement des symptomes cliniques, les modifications EEG sont dejaevidentes dans les deux cas (respectivement 1 an et 5 ans) avant le debut des symptomes cliniques. Dans la sulfatide-lipidose, les caracteristiques EEG traduisent des particularites qui sont manifestement differentes de celles vues dans d'autres erreurs innees connues du metabolisme, frappant le systeme nerveux central avant sa pleine maturation. Les auteurs suggerent que des investigations EEG repetees au cours de maladies neurologiques obscures, chez de jeunes enfants, peuvent apporter un signe precoce de diagnostic differentiel. ZUSAMMENFASSUNG EEG Untersuchungen bei 22 Kindern mil Sulphatid-Lipidose (Metachromatische Leukodystrophie) Es wird uber klinische und EEG-Untersuchungen bei 22 Kindern mit histologisch gesicherter Sulphatid-Lipidose berichtet. Das Auftreten der Symptome, der Ablauf der einzelnen Untersuchungen und der Verlauf der Krankheit wird im Vergleich zu den EEG Erscheinungen betrachtet. Bei zwei Kindern (Geschwister eines schon betroffenen Patienten) begannen die EEG-Untersuchungen vor dem Auftreten klinischer Symptome, und in beiden Fallen waren die EEG-Veranderungen bereits deutlich vorher ablesbar (ein Jahr und 5 Jahre vorher). Bei der Sulphatid-Lipidose finden sich EEG-Veranderungen, die sich von denen anderer angeborener StofTwechselstorungen mit Befall des ZNS vor der vollen Ausreifung wesentlich unterscheiden. Es wird erortert, das durch wiederholte EEG-Untersuchungen wahrend einer unbekannten neurologischen Erkrankung im jungen Kindesalter ein fruhes Zeichen in der Differential-diagnose gewonnen werden kann. RESUMEN Estudios electroencefalograficos en 22 ninos con lipoidosis sulfatida (leucodistrofia metacromatica) Se aportan los estudios clinicos y EEG realizados en 22 ninos con lipoidosis histologicamente probada. Se consideran en relation con la evolution del EEG el inicio de los sintomas, las diversas investigaciones realizadas y el curso seguido por cada caso. En dos ninos (hermanos de un paciente ya afecto) en que los estudios EEG empezaron antes del inicio de los sintomas clinicos, los cambios electricos eran ya evidentes en ambos casos (1 y 5 anos respectivamente) antes del inicio de los sintomas clinicos. En la lipoidosis sulfatida, los caracteres EEG muestran peculiaridades que son substancialmente diferentes de las vistas en otros tipos de error congenita del metabolismoafectando el SNC antes de su maduracion total. Se sugiere que las investigaciones EEG repetidas en el cur so de una enfermedad neurologica obscura en ninos pequenos, puede ofrecer un signo precoz de valor para el diagnostico diferencial.
