Wasp Stings with Multiple Organ Dysfunction

Objective:�Allergicreactionsfromwaspstingsusuallyoccurshortlyafterthestings.�Seriousandlesscommonsystemicmani- festationsincludingmultipleorgandysfunctioncanalsooccuratalatertime. Methods:�MedicalrecordsofpediatricpatientsadmittedbecauseofwaspstingsintotheFacultyofMedicineSirirajHospital� from�2000�to�2009�werereviewed.� Results:�Ninecaseswereadmittedbecauseofsystemicreactionsfromwaspstings.�Mostrequiredonlyintravenousfluidand� antihistamine.�Twoyoungchildrendevelopedmultipleorgandysfunctionmorethan�24�hoursaftermultiplestings.�Bothpatients� requiredartificialventilation.�Thefirstpatientreceived�20�daysofrenalreplacementtherapyandrecoveredcompletely.�The� secondpatientdevelopedsepsisduringperitonealdialysisanddiedonthe�34 thday.� Conclusion:�Cliniciansshouldbeawarethatyoungchildrenwithmultiplestingsareatriskofmultipleorgandysfunction.�We� suggesthospitalizationforatleast�48�hourstoprovideearlydetectionofcomplicationsandmanagement.
