For children with congenital central hypoventilation syndrome, and those patients with high cervical spine injuries and chronic respiratory insufficiency, this is indeed an alternative to long-term positive pressure mechanical ventilation. These individuals are usually mentally competent and hoping to lead an independent life with mobility. Diaphragm pacing offers important advantages to a carefully selected group of patients with respiratory paralysis. A thorough preoperative evaluation with wellplanned procedures for surgical implantation, and proper postoperative care will lead to a successful outcome. 2
