Differences in 11C-PIB-PET Imaging Patterns In Patients With Alzheimer’s Disease Without Microbleeds And Those With Cerebral Amyloid Angiopathy (P6.320)

OBJECTIVE: To clarify differences in 11 C-Pittsburgh compound B (PIB)-positron emission tomography (PET; 11 C-PIB-PET) imaging patterns in patients with Alzheimer’s disease (AD) without cortical/subcortical microbleeds (CMBs), and in those with cerebral amyloid angiopathy (CAA). BACKGROUND: PIB is a PET ligand that binds to β-amyloid in vivo. The 11 C-PIB-PET will become useful diagnostic imaging tool for both CAA and AD. However, AD is frequently accompanied with CAA. It is important to distinction of CAA and AD without CAA that we clarify differences in 11 C-PIB-PET imaging patterns in patients with AD without CMBs, and in those with CAA. DESIGN/METHODS: We examined 13 patients diagnosed with AD without CMBs based on clinical criteria (mean ± SD age, 70.8 ± 8.4 years; females, 11; males, 2; mean ± SD Mini-Mental Status Examination [MMSE], 17.4 ± 6.5), and 10 age-matched patients with clinically probable CAA (age, 68.9 ± 9.0 years; females, 6; males, 4; MMSE, 17.4 ± 7.8). We analyzed the standard uptake value ratio (SUV-R) of 50- to 70-min images of scans, divided the imaging data into 24 brain sections by using a 3D stereotaxic ROI template, and compared each image’s average of SUV-R between the patients with CAA and AD patients. The data were assessed by Mann-Whitney U-test, with statistical significance set at P < 0.05. RESULTS: Compared to patients with CAA, patients with AD without CMBs had significant higher average SUV-R in the frontal, parietal, and temporal lobes, and not lower in occipital lobes. The anterior/occipital ratio was not significant higher in patients with AD than in patients with CAA. This difference may be reflected by the pathological difference between AD and CAA.
