Analysis of Deployment Errors in Outpatient Pharmacy and Research of Interventions

Objective To investigate the prescription deployment errors in outpatient pharmacy of our hospital in the status quo,analyze the related factors and research the preventive measures against each factor in order to reduce the dispensing error rates and ensure the safety of medication.Methods Checking the prescription deployment in outpatient pharmacy randomly from January to February,2010,recording all kinds of deployment errors,and statistically analyzing various types.Results The total volume of prescriptions dispensed is 50 266,including 377 volume with dispending errors,taking up about 0.75% of total prescription volume.There are 3 main types of errors that are number error(260 volume,taking up about 68.96%),species errors(130 volume,taking up about 34.48%),and strengths errors(10 volume,taking up about 2.65%).The deployment errors occur in the staff with higher qualification are higher,which have little to do with the working time.Conclusion The deployment errors are related to the pharmaceutical packaging,attitude of the staff.Therefore,in order to prevent dispensing errors,it is very necessary to research the specific interventions to improve the accuracy of prescription deployment.
