Tight blood glucose control in prevention of surgical site infections during perioperative period of open cardiac surgery: a meta-analysis

OBJECTIVE To evaluate the effectiveness of tight blood glucose control in the prevention of surgical site infections(SSI)during perioperative period of open cardiac surgeries.METHODS A literature research was conducted in Cochrane Central Register of Controlled Trials,Pubmed,EMbase,CBM and CNKI.Randomized controlled trials comparing tight blood glucose control and conventional control groups in preventing SSI were collected and evaluated.The data were extracted from the including articles for meta-analysis.RESULTS Totally 7 RCTs involving 1140 patients were included in the study.Compared with the conventional control group,moderate tight blood glucose control group(150 mg/dl) could reduce the SSI rate [RR=0.09,95%CI(0.02-0.33),P=0.0003 and shorten the length of hospital stay MD=-2.71,95%CI(-2.78-2.63),P0.0001]as well;as compared with the highly tight blood glucose control group(130 mg/dl)with the conventional control group,the difference was not statistically significant.In addition,tight blood glucose control could reduce the new onset of atrial fibrillation [RR=0.70,95%CI(0.56-0.87)],the difference in the postoperative mortality was not statistically significant.CONCLUSION Moderate tight blood glucose control can reduce the incidence of SSI and shorten the length of hospital stay;and tight blood glucose control can reduce the new onset of atrial fibrillation,however it does not influence the mortality.
