Immunhistologische Analyse der chronisch-obstruktiven Sialadenitis - I. Veränderungen in der Expression von Antigenen des Haupthistokompatibilitätskomplexes*

Im Zuge der Elektrolytsialadenitis und der sich daraus entwickelnden obstruktiven Sialadenitis kommt es noch, bevor morphologisch fasbare Epithelveranderungen und lymphohistiozytare Entzundungsinfiltrate nachweisbar sind, zu Veranderungen in der epithelialen Ausstattung mit Antigenen des Haupthistokompatibilitatskomplexes (MHC), und zwar im Sinne einer Induktion von Klasse II-Antigenen und einer verstarkten Expression und Induktion von Klasse I-Antigenen. Diese Veranderungen beginnen fokal und sind lediglich, was die HLA-A, B,C- und -DR-Expression im Azinusepithel betrifft, mit dem Auftreten unmittelbar benachbart liegender Entzundungszellen assoziiert. Diese herdformigen Alterationen konfluieren in spateren Stadien der Erkrankung und sind in Arealen mit Fibrosklerose und Parenchymatrophie akzentuiert. Die veranderte MHC-Antigenausstattung des dem chronischen Sekretstau exponierten Epithels ist moglicherweise ein wichtiger Teil der postulierten „Immunreaktion am Gangepithel”, die diese Form der Immunsialadenitis initiieren soll. In the course of electrolyte sialadenitis progressing towards obstructive sialadenitis, changes in the epithelial expression of antigens of the major histocompatibility complex (MHC) occur even before morphological alterations and inflammatory cells can be detected. Class I antigens are enhanced and induced, and Class II antigens are induced. These changes begin focally. Only the expression of HLA-A,B,C and -DR in acinar epithelium is linked to the close vicinity of lymphohistiocytic infiltrates. In later stages of the disease the foci tend to fuse and antigenic changes are pronounced in areas of fibrosclerosis and parenchymal atrophy. The altered MHC antigen expression induced by chronical saliva congestion probably represents an important part of the postulated “immunoreaction in the duct epithelium” supposedly giving rise to this type of immunosialadenitis.
