The internist plays a vital role in managing the elderly patient undergoing emergency surgery. Internists are involved in each stage of care, not only as primary care physicians but also as hospitalists. As the primary care provider (PCP), the internist develops a relationship with the patient, is familiar with the medical history, and understands the patient’s preferences for treatment. When a patient suffers an acute event requiring hospitalization, it is often the internist who is charged with the management of care. Before pursuing surgery, the internist conducts a medical evaluation, including a risk assessment, to determine whether the benefits of surgery will outweigh the potential risks. Elderly patients often have comorbidities that may complicate postsurgical outcomes. The internist determines if the patient is too frail for surgery.
