Acromegaly is a known risk factor for ischemic heart disease. An adrenal adenoma along with increased urine catecholamine levels is highly indicative of pheochromocytoma. However, ischemic heart disease can cause increment in the urine catecholamines. Herein, we present a case having classic features of acromegaly and ischemic heart disease. On his thorax computed tomography scan performed for paracardiac opacity on X-ray, there was an adrenal adenoma and his urine catecholamine levels were high. After he underwent coronary by-pass grafting operation, his urine catecholamine levels returned to normal. If clinical probability of pheochromocytoma is low in the setting of increased urine catecholamines, other sympathic activation causes like ischemic heart disease, should be
