Coriorretinitis multifocal por lues en paciente VIH

espanolLa sifilis continua siendo una causa poco frecuente pero importante de uveitis. Con mucha frecuencia las uveitis sifiliticas han sido diagnosticadas y tratadas como uveitis idiopaticas, sobre todo si el paciente no presenta otras manifestaciones de sifilis secundaria. La sifilis ocular es la infeccion bacteriana intraocular mas frecuente en los pacientes VIH, y debe descartarse en toda uveitis en pacientes seropositivos, siendo en muchas ocasiones la primera manifestacion del VIH, independientemente del numero de CD4. Presentamos el caso clinico de un paciente VIH+ afecto de uveitis posterior que, tras la ausencia de mejoria con el tratamiento convencional y la positividad de la serologia luetica, fue diagnosticado de uveitis multifocal de origen sifilitico. Most times syphilitic uveitis are diagnosed and treated as being idiopathic, specially if the patient did not show another secondary syphilis features. It is a sex-transmitted disease which, in the last decade, has raised its incidence due to the growing HIV positive population, subtherapeutic antibiotic dosage, and lack of knowledge of the disease. Ocular syphilis is the most prevalent bacterial intraocular infection in HIV-positive patients, and must be ruled-out in any uveitis of a immunocompromised patient. Sometimes it can be the first manifestation of HIV infection, independently of the CD4 recount. We present the clinical case of a seropositive patient with posterior uveitis. After failure of conventional therapy and positivity of serology, he was diagnosed as having a syphilitic multifocal chorioretinitis. Therapeutic keys of the disease are discussed.disease. In the treatment of cancer, targeting angiogenesis alone or in combination with chemotherapy has already become a clinical reality. The key role of angiogenesis in these proliferatives disorders explains the effectiveness observed with these targeted therapies.
