DISSECTING ANEURYSM OF THE ABDOMINAL AORTA DUE TO BLUNT TRAUMA

A 57-year-old man was crushed under his cultivator, immediately experiencing abdominal pain and lumbago. Ultrasonography showed an intimal flap in the abdominal aorta to be diagnosed as dissecting aneurysm of the abdominal aorta due to blunt trauma. Digital subtraction angiogram showed an intimal tear with dissection below the renal arteries expanding to the right external iliac artery. At the operation a transverese intimal tear was presented with abdominal aorta except for posterior wall.The dameged aorta was replaced with a bifurcated woven dacron graft from below the renal arteries to iliac arteries, including reconstruction of the inferior mesenteric artery. The postoperative course was uneventful and digital subtraction angiogram revealed a complete correction.
