The comparative study on endothelial function, insulin resistance and serum adiponectin in different glucose tolerance populations.

Objective To probe the changes of serum adiponectin level, insulin resistance and endothelial function in different glucose tolerance populations. Methods 183 subjects including 47 cases of impaired fasting glucose (IFG), 52 cases of impaired glucose tolerance(IGT), 41 cases of diabetes(DM) and 41 cases of normal glucose tolerance(NGT) were chosen from physical examination population aged 35-55 yrs without cardiovascular disease, dyslipidemia and other clinical diseases. Plasma glucose, insulin, serum adiponectin, insulin resistance index(HOMA-IR) and endothelium-dependent dilatation(EDD) were measured and compared with each other. Results There was no difference in fasting insulin level among IFG, IGT and DM, but they were significantly different with the NGT group. For the postprandial plasma insulin, there were significant difference between group of NGT and groups of IGT and DM, as well as IFG group and groups of IGT and DM, respectively. The HOMA-IR of DM group was higher than that of IFG, IGT and NGT group. Meanwhile, the serum adiponectin and endothelial function was decreased from NGT, IFG, IGT to DM group. EDD decreased with the decline of adiponectin level. Conclusion There are increase of HOMA-IR and decreases of serum adiponectin and endothelial function in IGT, IFG groups, and they are even significantly in DM group. Serum adiponectin is negative correlated with fasting plasma glucose and HOMA-IR, while positive correlated with EDD.
