Effect of parameters of ventricular rate and haemodynamics in patients with atrial fibrillation of rapid ventricular rate and heart failure by injection metoprolol

AIM To investigate the efficacy and safety of intravenous metoprolol in the treatment of patients with atrial fibrillation of rapid ventricular rates and heart failure,and the effect on parameters of haemodynamics.METHODS Patients with atrial fibrillation of rapid ventricular rates and heart failure received routine treatment amd observed for 0.5 h.If ventricular rate was above 100 beats·min~(-1) and blood pressure was above 100/60 mm Hg(1 mm Hg=0.133 kPa),we divided them into 3 groups.The first group was administrated metoprolol 10 nag by minipump in 1h;the second group was administrated metoprolol 5 nag in l0 min by injection directly,adding 5 mg if heart beats was still above 100 beats·min~(-1) and blood pressure was above 100/60 mm Hg after 10 min;the third group was administrated normal saline as the control group.If either ventricular rate was below 60 beats·min~(-1) or blood pressure was below 90/60 mm Hg,ex- periments were terminated.Symptom,physical sign,heart rate,blood pressure and parameters of haemodynamics were observed before injection and 1 h after injection of metoprolol or normal saline.RESULTS After injection of metoprolol, the clinical symptom and physical sign were improved significantly in majority of patients.The heart rate was decreased in minipump group and injection group significantly (P0.05).The systolic blood and diastolic blood pressure were de- creased but not significantly compared with control group (P0.05).In the parameters of haemodynamics,CO,SV,SI, LVET were increased significantly compared with the control group (P0.05).CONCLUSION Intravenous metoprolol is effective and safe in patients with atrial fibrillation of rapid ventricular rates and heart failure,and is able to improve the clinical symptom and physical sign,decrease the heart rate,improve the parameters of haemodynamics.
