Paroxysmal nocturnal hemoglobinuria (PNH), a myelodysplastic, hematopoietic stem-cell disorder, is characterized by an increased sensitivity to complement-mediated erythrocyte lysis [1]. Reported radiologic findings include enlarged kidneys, cortical thinning, and evidence of cortical infarcts and papillary necrosis [2]. Histologic examination of the kidneys typically shows deposition of hemosiderin [3, 4]. We report a case of PNH with distinctive magnetic resonance imaging (MRI) findings in the kidneys that we attribute primarily to heavy deposits of hemosiderin in the renal cortex.
