Failure types, consequences and possible remedies

Abstract The main objective of the paper is to make suggestions for failure prevention. An analysis of several types of engineering failures by components and industry and the consequences of failures reveals that main sources of failure originate from human error. In particular, the design process involving early conceptual stages, materials selection and planning for reliable production figures heavily in this. Some suggestions involving new approaches for education and training are advanced as a basis for better engineered products.
