Intervenção farmacêutica no processo da validação da prescrição médica

Objective: Analyze pharmaceutical interventions performed during validation of prescription and demonstrate the process of validation flow of the prescription in the studied hospital. Method: We conducted a single-centered, observational, descriptive and transversal study in a private hospital, Sao Paulo (SP). Pharmaceutical interventions performed during the validation process of the prescription were quantified and analyzed from January to December 2012. Data were collected from the information recorded in the electronic database of the Hospital’s Pharmacy. Pharmaceutical interventions were classified according to the categories of pharmaceutical interventions in standardized institution. Results: 78341 prescriptions were validated. The average number of prescriptions per month was 6528. The total of pharmaceutical interventions performed was 15878, an average of 1323 per month. The largest number of interventions was related to setting time of intake, dose adjustment, adjustment of intake route, prescription duplicate, medication duplicate and contraindicated medication. The percentage of interventions accepted by prescribers was 99.65%. Conclusion: The pharmaceutical interventions performed prior to dispensing medications are effective in the prevention of medication errors. The application of a validation system of prescription done by a pharmacist, ensures greater treatment safety and quality of a hospitalized patient.
