INFORMATION AND COMMUNICATION TECHNOLOGIES TO SUPPORT SELF-MANAGEMENT OF CHRONIC NON-COMMUNICABLE DISEASES

A primary focus of self-care interventions for chronic illness is the encouragement of an individual's behavior change necessitating knowledge sharing, education, and understanding of the condition. The use of the Internet to deliver Web-based interventions to patients is increasing rapidly.Whether Web-based technologies can improve disease self-management is uncertain. This paper aims to describe how the Web-based self-management support systems couples’ evidence-based behavioral change components (self-monitoring of symptoms, physical activity, and medication adherence) with real-time monitoring and feedback.The information and communication technologies aims to address the self-management needs of patients with chronic illness by improving access to disease information, strategies to manage symptoms and social support.
