Maintaining Pulmonary Rehabilitation Effects in COPD Patients at Home – A Protocol Study

Background: Pulmonary Rehabilitation (PR) is a multidimensional intervention that enables COPD patients to acquire greater autonomy and confidence, while enable the development of self-management competencies for maintaining an active lifestyle. However, PR remains highly inaccessible. With this study we intend to develop and assess effectiveness of a post-PR program at home. Methods: a home-based PR program will be implemented for 12 months, in a sample of COPD patients collected after a outpatient PR program, using strategies such as the diaries, pedometer and illustrated guide, and the follow-up will conduct with telephone calls. Healthcare professionals will be trained. New ways to anticipate and transfer the PR content, in a safe and supportive environment, to the home setting after completion the program, will be integrated in the initial PR program. A randomized controlled will be conducted, and the main outcomes will be: exercise capacity (1minute sit-to-stand test); disease’s impact (COPD Assessment Test); quality of life (Saint George Respiratory Questionnaire); motivational regulations (Behavioral Regulation Exercise Questionnaire -3) and physical activity (pedometer). Data will be collected at the end of PR program, at 3-, 6- and 12-months post-PR. Changes in the outcome measures will be compared between groups. Method: we hope that this study could improve COPD’s patient’s health condition, quality of life and awareness about positive health behaviors, through a follow-up strategy. This intervention will be conducted by trained nurses to be modulators of behavioral change, in order to teaching self-management skills and support in finding (new) suitable daily activities.
