The efficacy of noninvasive positive pressure ventilation in patients with obesity hypoventilation syndrome according to the android/gynoid fat ratio and position of ventilation

It has been shown that noninvasive positive pressure ventilation (NPPV) is successfully used in the treatment of respiratory failure in patients with obesity hypoventilation syndrome (OHS). However, the existing studies have not focused on body type The objective of our study was to determine if the android/gynoid fat ratio is positively correlated with prevalence of OHS and to develop a scheme of NPPV Prospective study in OHS patients performing NPPV over a period of 1 year. Patients were divided into 2 groups according to the type of fat ratio. We also compared the efficacy of NPPV in supine patients with those in the lateral position 82 patients were eligible for analysis with a mean body mass index 45,3+-7,7 kg/m^2. 43 patients were included in the group of android fat ratio (AFR) and 39 in the group of gynoid fat ratio (GFR). 42 patients were included in group of supine position and 40 in the group of the lateral position. PaCO2 in the AFR group was significantly higher than in the GFR group (P We conclude that the android fat ratio is closely associated with prevalence of OHS especially in women. NPPV in the lateral position requires smaller IPAP and EPAP increment versus NPPV in the supine position.
