A comparative study of 201 Tl, 123 I BMIPP myocardial SPECT with 18 F FDG PET for evaluation of viable myocardium

Objective To evaluate the value of 201 Tl, 123 I BMIPP myocardial SPECT for detection of viable myocardium via comparative study with 18 F FDG PET Methods The left ventricular myocardium was divided into 9 segments in 31 patients with acute myocardial infarction (AMI) Uptakes of 201 Tl and 123 I BMIPP were visually scored as follows: 0=normal uptake, 3=severe reduction, and the scores of two tracers were compared with each other in each segment The 18 F FDG uptakes were determined visually and quantitatively in the segments with hypoperfusion detected by 201 Tl, 123 I BMIPP SPECT Furthermore, comparative analysis of 18 F FDG PET and 201 Tl, 123 I BMIPP SPECT findings with wall motion was also performed Results In 112 segments with hypoperfusion, segments of 201 Tl, 123 I BMIPP uptake match were 42.9% (48), and the mismatch were 57 1% (64) The 18 F FDG PET viable rate constituted 92 2% of segments with 201 Tl, 123 I BMIPP mismatch and 52 1% (33 3% of segments with severe reduction in uptake of 201 Tl and 123 [KG*7/9]I BMIPP) of segments with 201 Tl, 123 I BMIPP match As to the segments with akinesis~dyskinesis of wall motion, 65 6% were viable by the 18 F FDG PET, and 50 0% were viable by the 201 Tl, 123 I BMIPP SPECT Conclusions 201 Tl, 123 I BMIPP myocardial SPECT is useful for evaluating viable myocardium, but it may underestimate the viable myocardium as compared with 18 F FDG PET
