Infección por virus de inmunodeficiencia humana en el embarazo: Tratamiento anti-retroviral y vía de parto

Resumen La terapia anti-retroviral utilizada durante el em-barazo en madres infectadas con VIH disminuye latransmision perinatal del virus. Esto es valido paramono, bi y tri-terapia (HAART), siendo mayor elefecto de esta ultima. Sin embargo, cuando se utili-zan dichas terapias se debe tener en consideracionriesgos potenciales para la madre e hijo (hiperglicemia,acidosis lactica, toxicidad mitocondrial, rash cuta-neo, dano hepatico, sindrome hipertensivo y partoprematuro).La operacion cesarea electiva reduce la transmi-sion perinatal del virus en pacientes sin terapia omonoterapia. Con tri-terapia este beneficio no estademostrado.Este articulo revisa la evidencia de efectos bene-ficos y adversos de la terapia anti-retroviral y de laoperacion cesarea y propone una pauta de manejode las pacientes infectadas con VIH en el embarazo.
