Medical and surgical lessons learned from a severe case of Fusarium solani keratitis

2018
This report explores the appropriate diagnostic and therapeutic measures for a complicated case of severe Fusarium solani keratitis resulting from a delayed diagnosis. Confocal microscopy, polymerase chain reaction, microbiologic, and pathologic tests were used to diagnose and define the pathogen. In this case of F solani keratitis, there was a chance that the eye would have to be enucleated. This was prevented in this patient by intensive medical and surgical care comprising 3 therapeutic keratoplasty surgeries with increasing graft size, 3 pars plana vitrectomies, 2 amniotic membrane transplantations, 27 anterior chamber washings with antifungal medications, and 1 corneal crosslinking surgery.
