Analysis on efficacy and influencing factors of retention enema with compound on ulcerative chronic radiation proctitis

Objective To evaluate the efficacy of retention enema with compound on chronic radiation proctitis (CRP) with rectal ulcer and to analyze its influencing factors. Methods A retrospective study was conducted to analyze the clinical data of 51 patients with ulcerative CRP in the Sixth Affiliated Hospital of Sun Yat-sen University from November 2011 to June 2018. The influencing factors of retention enema with compound were analyzed. And the therapeutic efficiency with different treatment time of it was compared. Results Among the 51 CRP patients with rectal ulcer, there were 18 patients treated effectively by retention enema with compound, and the effective rate was 35.3%. Compared with the patients ineffectively treated, there were more patients maintain treatment by retention enema with compound for more than 6 months among the patients effectively treated (10/18 vs. 2/33, P = 0.000) . And we also found higher telangiectasia scores[3 (1.5, 3) vs. 2 (0, 3) , P = 0.048], lower platelet levels[ (210.3±73.7) ×109/L vs. (252.9±64.8) ×109/L) , P=0.001] and lower leukocyte levels[4.7 (3.7, 5.8) ×109/L vs. 6.0 (4.6, 6.5) × 109/L] in the patients effectively treated. Multivariate analysis revealed that treatment time of retention enema was an independent influencing factor (OR = 0.074, 95%CI: 0.011-0.515, P = 0.009) . When treated by retention enema with compound on CRP with rectal ulcer, the effective rate of>6 months treatment was significant higher than that of ≤3 months treatment (83.3% vs.17.8%, P = 0.000) and>3 ~ ≤6 months treatment (83.3% vs. 27.3%, P = 0.022) . Conclusion Retention enema with compound had a certain effect on patients with ulcerative CRP. And as the treatment time of retention enema with compound extended to more than 6 months, its efficacy was much more significant. Key words: Chronic radiation proctitis; Ulcer; Retention enema
