Postinfarction left ventricular remodeling in patients with hypertension

1-3 years after myocardial infarction 140 patients (66 with - group 1, and 74 without - group 2 - hypertension before infarction) were examined by echocardiography. In group 1 concentric remodeling was found in 71.4% of patients with postinfarction scar occupying =25% of left ventricular area (nonextensive scar) and in 100% of patients with larger (extensive) scars. In group 2 concentric and eccentric remodeling was found in 43.8 and 6.3%, respectively, of patients with nonextensive and in 9.5 and 28.6%, respectively, of patients with extensive scars. These results allow to suggest that after myocardial infarction left ventricles of patients with preexisting hypertension undergo predominantly concentric remodeling while in patients without hypertension mainly eccentric remodeling takes place. These tendencies are more obvious in patients with larger postinfarction scars.
