A propósito del Parkinson: un plan funcional de cuidados multiprofesional en Atención Primaria

Is presented a case usual in primary care.  Elderly patient with advanced Parkinson's disease in addition to diabetes, hypertension, depression and osteoarthritis. Nursing leads a multidisciplinary and proactive care plan focused on the wishes and needs of the patient and her family. It includes techniques, health education, pharmacological review and social intervention. Primary care nursing is fundamental for the transit to the care at home, is need stability in them equipment and training to change the paradigm of cure by care.
