AN ADULT CASE OF BLEEDING MECKEL'S DIVERTICULUM IN WHICH PROSTAGLANDIN F2α STOPPED MASSIVE BLEEDING

A 26-year-old male was admitted to the Miyazaki Medical Association Hospital because of hematochezia. Blood transfusion was performed over 4 days to an amount of 2400ml to compensate for the blood loss. 99mTc-albumin scintigram demonstrated lower ileal bleeding. Meckel's diverticulum was suspected as a probable diagnosis. The bleeding was stopped completely by continuous infusion of prostaglandin F2α (5μg/min) for 9 hours into an ileocecal branch of the superior mesenteric artery. Diverticulectomy was performed after the diagnosis was established with Meckel's diverticulum by intestinal barium studies. The diverticulum was 3×7cm in size, and a part of the mucosa near the tip was erosive. Thus, in this case of Meckel's diverticulum presenting with intestinal bleeding, the preoperative diagnosis was successfully made by intestinal barium studies after the bleeding was stopped effectively with prostaglandin F2α.
