Adverse events of inter-hospital transport of critical pediatric patient in Hunan Children's Hospital

Objective To explore the causes of adverse events in process of inter-hospital transport and the solution to deal with it. Methods We collected the data of critical pediatric patient that admitted in Hunan Children's Hospital through inter-hospital transport from March 2014 to December 2014.We collected the adverse events of transport, and analyzed the possible reasons for adverse events. Results Three hundred and fifty-three critical pediatric patients were encrolled in this study, 21 of 353 children suffered adverse events during transport, 6 cases(1.71%) suffered the life-threatening adverse events, and 15 cases(4.25%) suffered severe adverse events.Adverse events happened in 19 male patient, and 2 cases was female, the incidence of adverse events was higher in male cases than that in female cases(P=0.03). Nineteen cases with respiratory and mixed system diseases suffered from adverse events.Three cases suffered emergency adverse events of trachea cannula exodus for the reason of inadequate sedation.Fifteen cases(4.25%) suffered from severe adverse events, mainly reasons for happening were inaccurate condition assessment and breakdown of equipment.Doctor who obtained advanced life support training certification had lower incidence of adverse events compared with those without advanced life support training certification[(3.09%(6/194 cases) vs.9.43%(15/159 cases)] (χ2=6.28, P=0.02). Conclusion Adverse events are common in process of inter-hospital transport.Male children get higher incidence rate than female.Patients with respiratory and mixed system disease get adverse events more often.Trachea cannula exodus is the main emergency adverse events.The reasons for adverse events contain inadequate sedation and failure of equipment, such as failure of oxygen and battery.Improving the skill of doctor may make contributions to reduce the incidence rate of adverse events.
