Effect of laminar air flow wards on distribution and resistance of pathogenic bacteria causing VAP in intensive care unit

2011
OBJECTIVE To investigate the distribution and drug resistance of pathogenic bacteriA of ventilator-associated pneumonia(VAP) in laminar air-flow and popular ICU wards, and to provide a scientific basis for the clinical reasonable application of antibiotics.METHODS The data of pathogenic bacteria isolated from VAP patients in different ICU wards were retrospectively analyzed.RESULTS The most common five pathogens in laminar air-flow ICU wards were same in popular ICU wards. They were Acinetobacter baumannii,Klebsiella pneumoniae,Staphylococcus aureus,Pseudomonas aeruginosa and Stenotrophomonas maltophilia. The constituent ratios of each kind of bacteria were 12.83%,5.13%,28.21%,28.21% and 6.41% in popular wards and 22.99%,14.98%,14.98%,12.83% and 6.42% in laminar air-flow wards respectively. The drug resistant rates of Gram negative bacilli increased in laminar air flow wards.Antimicrobial susceptibility rates of imipenem and meropenem to A.baumannii and P.aeruginosa were about 20.00%.Imipenem and Meropenem had the higher antimicrobial susceptibility to K.pneumoniae with the rate of 90.00%.Cefoperazone/sulbactam had the higher antimicrobial susceptibility to A.baumannii with the rate of 74.42%.The main pathogenic Gram positive bacteria were S.aureus,to which vancomycin, linezolid and teicoplanin were 100.00% sensitive.CONCLUSION The laminar air-flow wards do not influence the spectrum of pathogens, but they can change the constituent ratio.Multidrug resistant A.baumannii become the most common pathogen of VAP.
