Тактика лікування хворих на травматичні та запальні захворювання шиї

Considering the fact that infl ammatory and traumatic diseases of the neck are often complicated by severe pathology resulting in fatal outcomes, it is required to determine the terms and to specify the scope of required therapeutic measures for improvement of treatment effi cacy and for reduction of adverse complications and outcomes. 48 patients with the following various complicated infl ammatory and traumatic diseases of the neck underwent treatment: chemical burns of the esophagus and laryngopharynx, infected penetrating wounds of the neck with the damage of larynx and pharynx, blunt neck traumas with laryngeal cartilage tears, esophageal foreign bodies, parapharyngeal abscesses. Based on the accumulated clinical experience, the following algorithm of surgical urgent measures has been suggested: restoring the integrity of the damaged organ (suturing laryngeal cartilages, removal of the esophageal, pharyngeal, laryngeal foreign body); restoring breathing (performing tracheostomy restoring feeding (tube feeding); drainage of the infl ammatory focus (lancing of the parapharyngeal abscess, neck phlegmon, cervical mediastinotomy. Postoperative management of infected wounds should be conducted via the open technique. Compliance with this treatment strategy made it possible to achieve recovery in 81,6% of patients.
