Protez kapak trombüslerinde tedavi yaklaşımları

Bu calismada, 2002-2011 yillari arasinda Ankara Universitesi Tip Fakultesi Kalp ve Damar Cerrahisi Anabilim Dali'nda protez kapak trombus tanisi konulan hastalar (89 hasta) retrospektif olarak cerrahi tedavi alanlar, trombolitik tedavi ve her ikisini birden alanlar karsilarstirildi. Yasayan ve mortalite izlenen hastalar karsilastirildi. Trombolitik tedavi basarili ve basarisiz olan trombolitik tedaviler ve bunlarin tedavi sonrasi kacina REDO ameliyati, kacina PANNUS/Trombektomi giderilmesi yapildigini karsilastirildi.Calismamiza dahil ettigimiz hastalarda Mitral kapaktaki trombozlarin Aort kapaktaki trombozlardan daha buyuk oldugu goruldu. Ayrica Mitral kapak trombozu olan hastalarin ikinci kez cerrahi gereksinimleri Aort kapakta trombozu olan hastalara gore daha fazla oldugu goruldu.Bu nedenle trombolitik tedavinin yaninda ve sonrasinda uygulanacak cerrahi tedavinin, hastaligin tedavi sansini arttiracagi gorusundeyiz.AbstractThis study is concerned with the management of trombosis formation of the prosthetic valves. The database was taken from the patients were operated between 2002-2011 period at University of Ankara, School of Medicine, Department of Cardiovascular Surgery. The sample material is composed of 89 patients. The patients?sample is divided to three categories: The first is consisted of the patients who recieved the medical (thrombolitic) treatment. The second is consisted of the patients who recieved surgical treatment. The third one is consisted comparision between both treatment. The comparision was made according to the mortality rate among the categories. The comparision was made according to the success of the medical treatment and after treatment, how many patients required REDO operation and how many patients has needed excision of PANNUS/Thrombectomy.The thrombosis on the mitral valve are larger than the aortic one, was detected in this study. The requirement of a second surgical attempt to the patient with mitral valve thrombus are higher than the aortic one?s.As a conclusion; our decision is that increment at the chance of treatment is depended to the surgical treatment at the time of or after the thrombolitic treatment.
