Patients with asymmetric sensorineural hearing loss often present a diagnostic dilemma. Magnetic Resonance Imaging (MRI) is considered the gold standard in the assessment of asymmetric hearing loss. The aim of this study was to analyze clinical and audiometric factors and their relationship with pathological changes in MRI in patients with asymmetric hearing loss. A retrospective analysis of the clinical files of patients undergoing brain and inner ear MRI was carried out between 2019 and 2017 at the Pedro Hispano Hospital, focusing on the reason for request, symptoms and audiometric study. 277 medical records were analyzed. Approximately 12% of patients had significant pathology on MRI. Of these, 51% had an asymmetry greater than 20% in speech discrimination and 36% unilateral tinnitus. Thus, patients with these changes are more likely to have pathological changes on MRI.
